What Happens If You Travel Without Insurance? Real Scenarios

The Short Answer

You pay every bill yourself, and no government will step in. The UK Foreign Office puts hospital treatment and evacuation for a broken leg in Spain at GBP 25,000 or more, and a stomach bug treated in a US hospital at GBP 150,000 or more. Foreign hospitals may demand immediate cash payment. A Schengen visa is also refused without EUR 30,000 of cover. Travl's AXA-issued insurance starts from AED 30.

What Happens If You Travel Without Insurance? Real Scenarios

If you travel without insurance and something goes wrong, you pay the full bill yourself, and no government steps in to cover it. The UK Foreign Office puts hospital treatment plus evacuation for a broken leg in Spain at GBP 25,000 or more, and a stomach bug treated in a US hospital at GBP 150,000 or more. For UAE residents, the fix costs less than a meal out: Travl travel insurance issued by AXA starts from AED 30 for Schengen trips.

  • A travel insurance policy that covers emergency treatment, hospitalisation and repatriation, bought before departure
  • Written declaration of any existing medical condition or pending tests, so related complications are covered (UK Foreign Office)
  • Confirmation that medical evacuation and a medical escort are included (Government of Canada)
  • For a Schengen visa, a certificate showing EUR 30,000 minimum cover valid across all Schengen states (EEAS)
  • The policy document and the insurer's 24-hour assistance number saved on your phone

Who pays your hospital bill if you have no travel insurance?

You do, in full, and the money is owed whether or not you can afford it. The Government of Canada tells its citizens outright that it will not pay their medical bills abroad, and that a provincial health plan may cover none, or only a small part, of the cost of care overseas.

The UK position is the same. The Foreign Office warns that without appropriate insurance you could be liable for emergency expenses, including medical treatment, which may cost thousands of pounds.

UAE residents are in a weaker position than either. Your employer's health card is built around treatment inside the UAE, and unless the policy document states worldwide cover, assume it stops at the border.

What does a hospital abroad actually do when you cannot pay?

Many hospitals ask for money before treatment starts. The Government of Canada notes that hospitals and clinics in other countries can be very expensive and may require immediate cash payment.

In practice that means a deposit on your card at admission, then a running bill for scans, surgery, the ward and medication. Without an insurer's guarantee of payment, the hospital has no one else to bill.

An insurer changes this. With travel medical insurance from Travl, the AXA assistance line can deal with the hospital directly, and you deal with getting better.

Scenario one: you break a leg in Spain

A simple fall on a Barcelona pavement becomes a five-figure problem. The UK Foreign Office puts the cost of hospital treatment and possible medical evacuation or repatriation for a broken leg in Spain at GBP 25,000 or more.

The treatment itself is only part of that figure. A cast means you cannot fly in a normal seat on the original date, so you pay for a new flight, extra hotel nights and possibly a medical escort.

The same trip with Spain visa insurance from Travl costs from AED 30 for cover, with EUR 30,000 of medical protection issued by AXA. That is the difference between a bad holiday and a debt you carry home to Dubai.

Scenario two: a quad bike crash in Greece

An activity accident is the most expensive kind of holiday injury short of intensive care. The UK Foreign Office lists a quad bike accident in Greece needing surgery and evacuation at GBP 80,000 or more.

There is a second catch here. Motorised activities and adventure sports sit outside many standard policies, so even an insured traveller needs to check the policy wording before renting a bike.

Read our guide to adventure sports travel insurance before booking anything with an engine, and confirm the activity with Travl by email or WhatsApp if the wording is unclear.

Scenario three: a stomach bug in the United States

The US is where uninsured travel goes from painful to ruinous. The UK Foreign Office quotes GBP 150,000 or more for a stomach bug or infection treated in a US hospital, with possible evacuation or repatriation on top.

That is not a rare injury or a surgery. It is a common illness that needs a drip and a few nights of observation, priced at US hospital rates.

For any trip to the US or Canada, Travl's international travel insurance starts from AED 70 and carries EUR 80,000 or more in medical cover. UAE residents heading there regularly should look at the annual multi-trip plan from AED 245 instead.

Scenario four: you need to be flown home

Getting home after a serious illness is a separate bill from the treatment, and it can be the larger one. The Government of Canada advises that a policy should cover medical evacuation to your home country or the nearest place with medical care, plus the cost of a medical escort.

Without insurance, an air ambulance or a stretcher booking on a commercial flight is paid by you or your family, in cash, before departure. The same source also covers the worst case: a policy should include repatriation in case of death, including preparation and return of remains.

Our post on medical evacuation and repatriation cover explains how this works under an AXA policy sold through Travl.

Scenario five: you apply for a Schengen visa without insurance

Without insurance your Schengen visa application is incomplete and will not be accepted at the counter. The European External Action Service lists travel medical insurance as a required document: it must cover the entire stay, be valid for all Schengen states, and carry a minimum of EUR 30,000.

The European Commission spells out what that insurance must cover: emergency medical care, hospitalisation and repatriation, including in the event of death. The same page sets the standard visa fee at EUR 90 for adults and EUR 45 for children aged 6 to 12.

Travl's Schengen visa insurance is built to this rule, accepted by VFS Global and BLS International, and emailed within minutes. If you need help with the application file itself, VisaWadi handles Schengen visa assistance for UAE residents from AED 299.

Why do uninsured travellers, and some insured ones, end up paying anyway?

The avoidable reasons are almost always the same four. Each one turns a covered event into a bill you pay yourself.

  • No policy at all. The most common reason, and the only one where the outcome is certain: every dirham of treatment, evacuation and rebooking is yours.
  • An undeclared condition. The UK Foreign Office warns that failing to declare an existing condition or pending treatment may invalidate your travel insurance. Read our guide to pre-existing conditions and travel insurance before you buy.
  • Alcohol or drugs. The same Foreign Office guidance notes that most policies do not cover events that happen after excessive alcohol or recreational drug use.
  • Relying on a home health card. A UK GHIC or EHIC does not cover private treatment, repatriation or mountain rescue, per the Foreign Office, and a UAE employer health card is even less use abroad.
  • Buying after the trip has started. Cover has to be in place before you leave the UAE; a policy bought from a hospital bed will not pay for the reason you are in it.

Our article on travel insurance exclusions walks through the rest of the fine print.

Visa service centres may add their own handling fee on top of the visa fee, as the European Commission notes. Travl's insurance price is separate from both and does not change with the embassy you apply to.

What should a UAE resident do before the next trip?

Buy cover before you book anything else, then match the plan to the trip. Schengen trips need the EUR 30,000 minimum, long-haul trips to the US or Canada justify the higher medical limit on the international plan, and anyone flying more than twice a year saves with an annual policy.

Compare all travel insurance plans for UAE residents on one page, get a quote in minutes, and keep the policy PDF on your phone. If you are unsure which plan fits, our guide on single trip versus annual travel insurance settles it, and the team is on email or WhatsApp for anything the guide does not answer.

No. No government covers its residents' medical bills abroad, and a UAE employer health card is built for treatment inside the country. Without a travel policy that states worldwide cover, the hospital bills you directly, and it may ask for a deposit before treatment begins.
No. The European External Action Service lists travel medical insurance as a required document, with a EUR 30,000 minimum, valid for the whole stay and in all Schengen states. Travl's AXA-issued Schengen visa insurance meets this from AED 30 and is accepted by VFS Global and BLS International.
GBP 25,000 or more for a broken leg in Spain, according to the UK Foreign Office, once hospital treatment and possible medical evacuation are added up.
